There have been scattered reports of physicians and other LIPs being unable to complete Discharge Med Rec in Soarian. They may be intermittently “kicked out” of Admission Med Rec or Discharge Med Rec. There is a ticket opened with Siemens. They believe they know what the problem is but they do not have a fix yet.
When this happens, the MRC Manual Medication Reconciliation report will need to be run manually from Soarian, and completed by the LIP. Many Physicians/Practitioners will not be familiar with this process, and may need assistance. Below is a tip sheet outlining the steps to complete the Medication Reconciliation when the technical issue will not allow it to be completed electronically.
